Hospice Levels of Care

At Suburban Hospice, we provide a continuum of care that depends on the needs of our patients and families.
- Routine home care is provided in your home.
- In-patient respite care is provided in a hospital or extended care facility
- General inpatient care is provided when a patient’s symptoms require interventions that cannot be offered in the home.
In general, home hospice care is the best for most patients, and the majority of our patients are cared for in the home. In some cases, caring for a loved one at home is not possible. We also provide hospice care in nursing facilities and assisted living.
Sometimes, when symptoms cannot be controlled at home, a brief hospitalization is needed to get relief. Our affiliated hospitals can provide us with temporary stays for that purpose. Your hospice team must arrange these stays.
Caregivers who need a break can take advantage of a five-day respite stay in one of our area skilled nursing facilities. Respite care is an essential piece of caring for a loved one as it allows family and caregivers time to rest and recharge physically, mentally, and emotionally.
At Hancock Health in Greenfield, Suburban Hospice offers both respite and general inpatient care on the seven-bed unit designed to provide expert end-of-life care in a homelike setting. Although this setting is not an option for long-term stays, patients may need a few days for symptom control before transitioning back home or to another care setting.
