We provide hospice care at home, in private care facilities, assisted living, extended care facilities, and hospitals in Greenfield, Noblesville, and Lebanon and the surrounding areas.

If you have questions about hospice or would like to explore whether you or your loved one qualifies for hospice care, please follow these steps:

For New Patients

• Daytime: Call our toll-free number (833) 888-7222 during the day and press option 1 to speak to our clinical services manager about hospice care. If no one answers, please leave a message and we will call you right back.
• After hours: If you need to reach us after normal working hours (8 am – 4 pm), call our toll-free number (833) 888-7222 and leave a message. If you are calling after hours and it is an emergency, please press 0 instead of leaving a message and your call will be routed to the hospital switchboard and let them know you need to speak with the hospice nurse on call. (if you have trouble getting through on that number you can call the switchboard directly at (317)468-5544.) The operator will take your callback number and contact the hospice nurse immediately. We have a nurse on call 24/7 hours a day, so you can always reach us when you need us.

For Current Patients and Families

• Daytime: Call (833) 888-7222 and press option 1 to speak to our clinical services manager. If you need to leave a message, we will call you back promptly.
• After hours: Call (833) 888-7222 and leave a message (no need to press any key). If you need to reach the nurse on call right away, press 0 and leave your number with the hospital switchboard operator. The operator will contact the hospice nurse on call and she will call you right back. (If you are unable to get through on this number, please call the Hancock hospital switchboard directly at 317-468-5544.)
