For Admission

For admission to hospice, we will need:

• Contact information for patient
• Clinical notes to support the patient’s terminal diagnosis
• A physician’s order for hospice evaluation and treatment

For Consultation

If you have a patient who you think might qualify for hospice care, you can contact us without an order and we will be glad to go visit patient and family and do a consult. We will let you know promptly whether the patient is eligible for hospice.

In all cases, we will respond promptly, typically the same day we receive your referral.
